Repair Authorization & Terms
Authorization to Perform Work
I authorize Cascade Brake & Alignment ("CBA") to inspect, diagnose, test, and repair my vehicle as described on this repair order. I authorize CBA employees to operate the vehicle for diagnostic, testing, inspection, and verification purposes, including road testing when necessary.
No repairs beyond those approved by the customer will be performed without additional authorization.
Diagnostic Services
Diagnostic charges are based on technician time, expertise, and equipment used during the inspection and testing process. Diagnostic fees are earned regardless of whether a repair is ultimately performed or a specific failure is identified.
Additional Repairs
During the course of inspection or repair, additional conditions may be discovered that require further repairs. Customer authorization will be obtained before any additional work is performed.
Alignment, Suspension, and Steering Disclaimer
Vehicle alignment specifications may be affected by worn, damaged, bent, seized, rusted, modified, aftermarket, or collision-damaged components.
CBA will make reasonable efforts to adjust alignment angles within manufacturer specifications whenever possible; however, some vehicles may not be capable of achieving factory specifications due to existing vehicle conditions.
Customer understands that alignment adjustments alone may not correct concerns related to tire wear, tire pull, vibration, steering feel, handling characteristics, or drivability issues when those concerns are caused by factors outside the alignment system.
Brake System Disclaimer
Brake system performance depends on the condition of all related components. Repairs performed by CBA are limited to the components approved by the customer. Existing wear, damage, or defects in components not replaced may affect overall system performance.
Pre-Existing Conditions
Customer acknowledges that the vehicle may contain pre-existing mechanical, electrical, structural, corrosion-related, or cosmetic conditions that are not readily visible during inspection.
CBA is not responsible for failures resulting from pre-existing conditions, age-related deterioration, rust, corrosion, prior repairs, modifications, or unrelated component failures occurring during or after service.
Customer-Supplied Parts
CBA may decline installation of customer-supplied parts. If customer-supplied parts are installed, CBA provides no warranty on those parts and is not responsible for defects, delays, fitment issues, or failures related to customer-supplied components. Labor charges remain the responsibility of the customer.
Warranty
Unless otherwise stated in writing, warranty coverage applies only to repairs and parts supplied by CBA and is limited to the terms of the applicable manufacturer and/or shop warranty.
Warranty coverage does not apply to abuse, misuse, racing, off-road use, accidents, modifications, environmental damage, or failures unrelated to the original repair.
Storage and Vehicle Pickup
Customer agrees to promptly retrieve the vehicle upon notification that repairs are complete.
Vehicles left more than 7 days after notification may be subject to storage charges. Storage fees and vehicle lien rights may be exercised as permitted under Oregon law.
Payment
Payment is due upon completion of repairs unless other arrangements have been made in advance. Customer grants CBA a mechanic's lien on the vehicle to secure payment as provided by applicable Oregon law.
Personal Property
CBA is not responsible for loss of or damage to personal property left inside the vehicle.
